Accident Reports


Watch this video on accidents at work:


Choose one of the incidents on the video and write a brief report of the accident to be submitted to the Manager at the Human Resources.  Your report must include the following, but you may also need to make up some details:

1. Name, occupation, and sex of injured worker.
2. Place and date/time of the accident.
3. Description of how the accident happened.
4. Immediate causes of the accident
5. Contributing causes
6. Witness(es) – name and department.
7. Corrective action taken – when.
